Specialist weight management is an assessed care pathway. Eligibility, scope, duration and support depend on the service and assessment. Review the programme information and discuss suitability with the care team before enrolling.

Ask the service team about referral requirements. Share referral information only through the secure route provided. A referral does not itself confirm acceptance, funding or an appointment.

Confirm eligibility, authorisation and funding limits with the service team and payer before booking. Employer, insurer or public funding applies only where an arrangement is confirmed. Listing a programme does not establish funding.

Organisation care is agreed with the organisation. Check eligible members, services, funding limits and access arrangements. Funding care does not give an employer general access to clinical records.

Read the selected plan for member limits, included care and billing. Each recipient has distinct care needs and access permissions. Family membership alone does not permit an adult to view another adult's clinical information.
