Support assessment, patient education, follow-up and coordinated care.
About the role
Help patients navigate care and manage their health through nursing consultations, education and follow-up. Work with the wider clinical team, recognising changes in a patient’s condition and escalating concerns appropriately.
What you’ll do
- Conduct nursing assessments and triage within an agreed scope and approved protocols.
- Provide clear health education, medication support and self-management advice within your competence.
- Support chronic condition monitoring, prevention and follow-up after consultations or investigations.
- Recognise deterioration, safeguarding concerns and red flags, and escalate promptly.
- Coordinate care with clinicians and service partners and maintain accurate nursing records.
- Contribute to clinical quality and a respectful, accessible patient experience.
What you’ll bring
- Recognised nursing qualification and current registration in the relevant jurisdiction.
- Relevant clinical nursing experience and competence in assessment and care planning.
- Ability to recognise the limitations of remote assessment and escalate safely.
- Strong patient communication, documentation and digital working skills.
- Commitment to safeguarding, confidentiality and continuing professional development.
Helpful additional experience
- Experience in primary care, community nursing or chronic disease management.
- Telehealth, care coordination or patient education experience.
Working arrangements
Medocta is preparing for launch and developing its multidisciplinary clinical services. Digital consultations and follow-up are central to our approach. The country of practice, remote or in-person requirements, hours, engagement type, start date and remuneration will be discussed during recruitment and confirmed before appointment.
