About the role
Support medication optimisation, patient education and medicines safety within your professional scope. Collaborate with prescribing clinicians and other professionals to review treatment and improve continuity of care.
What you’ll do
- Review medication histories, adherence, potential interactions and patient concerns.
- Provide clear medicines education and recommendations to patients and treating clinicians.
- Support medication reconciliation and appropriate follow-up for long-term conditions.
- Prescribe only where you hold the required authorisation and within your competence.
- Identify medication-related risks and escalate concerns appropriately.
- Document reviews and contribute to medicines safety and clinical protocols.
What you’ll bring
- Recognised pharmacy qualification and current registration in the relevant jurisdiction.
- Relevant clinical pharmacy or patient-facing medicines review experience.
- Sound knowledge of medicines safety and professional scope limits.
- Clear communication and collaborative clinical working skills.
- Accurate documentation and commitment to confidentiality and continuing development.
Helpful additional experience
- Experience in primary care or long-term condition medication reviews.
- Additional prescribing qualification where relevant to the agreed role.
Working arrangements
Medocta is preparing for launch and developing its multidisciplinary clinical services. Digital consultations and follow-up are central to our approach. The country of practice, remote or in-person requirements, hours, engagement type, start date and remuneration will be discussed during recruitment and confirmed before appointment.
