About Our Tier 3 Weight Management Service
Our comprehensive program provides specialized support for adults with complex obesity, combining clinical expertise with personalized care to help you achieve sustainable weight management.
Access to a team of healthcare professionals including dietitians, psychologists, and exercise specialists.
Tailored treatment plans addressing your specific health needs, goals, and challenges.
Nutritional advice, physical activity guidance, psychological support, and lifestyle coaching.
Eligibility Criteria
- Be over 18 years old and registered with a Humber and North Yorkshire GP and referred by a GP
- Have a BMI ≥35kg/m² with any one of these comorbidities: Hypertension, Hyperinsulinaemia, Dyslipidaemia, Type 2 diabetes, Psychosocial dysfunction, Exacerbation of conditions such as asthma
- OR have a BMI ≥40kg/m² without comorbidities
- Have demonstrated that you have maximized primary care and community conservative management
- Have an understanding of the commitment required for a Tier 3 programme and are willing to engage with all aspects of the program
- Patients with a BMI ≥50kg/m² may be referred directly without evidence of compliance with Tier 2 services
Exclusion Criteria
- Have a BMI less than that stated in the inclusion criteria
- Have a diagnosed eating disorder, disordered eating, or complex mental health issues
- Are referred from an inpatient mental health unit
- Are unable to participate in ALL components of the program
- Require weight loss injections alone or require Very Low Calorie Diets
- Have a greater nutrition risk (e.g., receiving cancer treatment or palliative care)
- Are pregnant
- Have a diagnosis of active Inflammatory Bowel Disease
- Have had previous bariatric surgery
- Have previously been referred and accepted onto a tier 3 weight management service
- Consume more than 20 units of alcohol per week
- Are under investigation for cardiac disease
Important Note on BMI Thresholds
For people from South Asian, Chinese, Black African or Caribbean family backgrounds, the BMI thresholds are reduced by 2.5kg/m² due to the higher risk of complications at lower BMI levels, in accordance with NICE Clinical Guideline CG189.

How to Access Our Service
The Tier 3 Weight Management Service is available by referral only. Follow these steps to begin your journey.
Discuss your weight management concerns with your GP, who will assess if you meet the criteria for the Tier 3 service.
If appropriate, your GP will complete and submit a referral to our Tier 3 Weight Management Service.
Once we receive and process your referral, our team will contact you to arrange your initial appointment.
What to Expect from the Program
The Initial Assessment
Your journey begins with a comprehensive assessment by our multidisciplinary team to understand your health status, weight history, and specific needs.
Your Personalized Plan
Based on your assessment, we'll create a tailored plan that may include nutritional guidance, physical activity recommendations, psychological support, and behavior change strategies.
Ongoing Support
You'll receive continuous support through regular appointments with our healthcare professionals, group sessions, and educational resources.
Progress Review
At the 6-month mark, we'll review your progress together. Meeting your weight loss goals at this stage is important to continue treatment through to 12 months.
